Hazel Choi BHK, MOT, CWCE
Registered Occupational Therapist
Hazel graduated from the University of British Columbia with a Master of Occupational Therapy in 2009. Previously, she completed a Bachelor of Human Kinetics in Physical Education at the University of British Columbia in 2006. Prior to working with Re: Function Health Group Inc., Hazel was employed as a Staff Occupational Therapist in an interdisciplinary outpatient return-to-work rehabilitation clinic. In this practice setting, Hazel was responsible for the assessment, treatment and provision of discharge recommendations for injured workers, as well as case coordination, conducting job site visits/job demands analyses, job coaching, and graduated return to work planning and monitoring. In addition, Hazel facilitated vocational and disability management workshops for clients with physical, cognitive and/or affective disabilities and disorders, and served as a fieldwork educator for students in the Master of Occupational Therapy program at the University of British Columbia.
Hazel’s current practice with Re: Function Health Group Inc. includes performing ergonomic evaluations, workplace assessments, job demands analyses, and return-to-work planning and monitoring for clients with musculoskeletal injuries.
Outside of work, you can find Hazel training at her CrossFit gym, enjoying the diverse restaurant options in the city, or planning her next travel adventure.
